Site Preparations *

SMP Accessibility

SMP Accessibility: Ensure that all SMPs are fully constructed and stabilized and will be accessible at the time of inspection. Locate and ensure accessibility to all basin cleanouts and observation wells specified on the PWD approved plans.

Erosion & Sedimentation (E&S) Controls and Site Stabilization

Erosion and Sedimentation (E&S) Controls and Site Stabilization: Once adjacent site areas are fully stabilized with vegetation or alternative measures, remove all E&S controls from on-site areas and from nearby City inlets in the street.

Traps/Hoods

Traps/Hoods: Ensure all required traps/hoods are installed and affixed flush against the structure’s side wall. Traps are meant to keep floatable debris from entering the pipes; gaps along the side of the trap will not pass final inspection.

Structures Cleaned

Structures Cleaned: Ensure all stormwater-related structures including, but not limited to inlets, pre-treatment structures, and connected piping, are cleaned of all trash, sediment, and debris accumulation from the construction process. Pre-treatment structures with excess debris and/or sediment may not pass final inspection.

Project Close-Out Documentation *

The applicant is advised to review Appendix M. PCSMP Project Closeout in the Stormwater Management Guidance Manual regarding how to complete the project closeout process.

These items will be reviewed by PWD and incorporated as feedback into the Post-Construction Stormwater Management (PCSM) Final Inspection Report.

Fees

Fees: Confirm that all outstanding fees have been paid.

Construction Certification Package (CCP)

Construction Certification Package (CCP): Confirm that a completed CCP has been submitted to PWD via the Project Dashboard.

Operations and Maintenance (O&M) Agreement

Operations and Maintenance (O&M) Agreement: Confirm that an up-to-date O&M Agreement reflecting the constructed conditions has been executed and recorded against the property.

Record Drawings

Record Drawings: Confirm that a red-lined Record Drawing has been submitted to PWD via the Project Dashboard. Refer to the Record Drawing Requirements and the Appendix K. Record Drawing Sample for more information.

Inspection Availability *

During the final inspection, PWD will need access to the interiors of manholes, outlet structures, cleanouts, and observation wells. Applicants must have the necessary personnel and tools available to open structures. Representatives from the contractor responsible for the construction and ownership are required to attend.

Please provide three different dates, at least 7 days from the date this form is submitted, and the times on each of those dates (between 8am and 4pm), available for the final inspection.
